Payment Information:
Guarantor hereby agrees to pay Seymour Hospital the full amount of all charges or any remaining balance, after all insurance payments have been received, on their account for medical services provided for them.
Seymour Hospital is not responsible for unpaid claims due to the guarantor not providing Seymour Hospital with proper insurance information or when the insurance information is not furnished to Seymour Hospital in a timely manner (by insurance filing deadlines).
